its actually pretty much up to you. i have 4 spread evenly on the underside. the piercer will tell you if the placement you choose could cause extra problems.

How far down depends on how many piercings you get. You can do a Jacob-s Ladder with 2, 3, 4, 5 or 6 piercings. Obviously the more you have the further down they go. Personally, if I were to get one of these I would have the first one start past the frenulum (because that part is very sensitive) and get 3 or 4 max. From what I-ve heard, this is one of the least painful genital piercings for guys (though still somewhat painful) because there are less nerves on the underside of the penis (again, except for the frenulum). Prince Albert, reverse PA and frenulum piercings are going to hurt worse.
