An oily waste material called smegma is generated and collects under the foreskin, particularly in the coronal sulcus (groove) just behind the glans (head).
You need to fully retract your foreskin at least twice a day (plus after any strenuous exercise such as sports, gym or swimming) and wash this away with soap and water. Water alone will not fully remove it because of its oily base.
Failure to regularly wash away the smegma will result in nasty smells and eventually in bacterial or fungal infections which grow easily on stale smegma.
